Booking Interviews

Hello All

It would appear that the interview booking process has changed at some point in a recent upgrade so now when pressing the activity button to record the date time of an interview this overly complex screen is displayed and the team are not sure what to put into the boxes and it doesn't seem to be saving the record.
Apparently it used to just display a date and time field that was much more straightforward.

I have attached a screenshot of how it now looks. :?
